PROCESS OF INVESTING
NEW ACCOUNT
Step 1
- Fill out and submit Forms Nos. 1 to 4

Step 2
- Go to any BPI Branch or Metrobank Branch to deposit your check/cash over the counter using payment slip or you may transfer online.
Step 3
- Email the following documents to support@affinitycorp.net
- 1 valid ID with visible photo and signature of the principal and co-investor, if any
- Forms Nos. 1 to 4
- Proof of fund tranfer or deposit
REMINDERS
NEW ACCOUNT
- You may open an account or several accounts under your name. However, it is advisable to maintain one account only for easy monitoring.
- Minimum account opening requirements are:
- Email address
- Mobile Number
- If with Co-investor or Co-Account holder, fill-out the spaces provided.
- Please follow Steps 1 to 3.
SUCCEEDING OR TOP-UP
Step 1
- Fill out and submit Form 01, Transaction Form

- Submit Proof of fund transfer of deposit

- Only the Principal or “Client 1” will fill up and sign the Transaction Form (Form no. 01).
- The Co-investor need not sign the Transaction Form.
